I have to say I'm extremely confused.
Yesterday I fully charged the phone, used it as normal during the day and MD was well down at around 10th in the list with only 1% used after 16 hours. This was just after the latest version of MD had installed.
Today I fully charged the phone, used it as normal and MD is top of the list with 10% usage after just 15 hours!
This is so odd, what is going on?
I didn't keep a record for the day when all appeared 'normal'.How much % of battery was used during this period of time in total by all apps?

MacroDroid 10% used for 11hrs, 47mins
Play services 4%

Location history 2%
Phone 2%
Photos 2%
Maps 2%
and so on.
On a 'good' day MacroDroid is well down the list with 1% used and I get 40/48 hours total.

Despite this, MD is at the top of the list with 6.3% consumption already. In addition it has 3h45m of background processing, which is nearly the whole time since the last full charge. What is the SOT in hours & % battery used.
What is the sleep time in hours and if available the deep sleep hours and for both % battery used.
Do you have this data?
From the data you have supplied it suggests the phone may not be deep sleeping when the screen is off.
I assume in all the testing you have undertaken that you do not open MD, so not writing, amending or checking any macros, otherwise MD battery consumption will increase.
It would be interesting to know the SOT hours when you get up to 48 hours battery life.
Finally, do you know the overnight battery drain?
I try to minimise this and typically strive for <2% for the ~ 8 hours I'm asleep.

I've had an interesting result recently.
A new version of Macrodroid was installed overnight whilst the phone was on charge.
The following day was a 'good' day in that MD was not even showing in the list of apps using battery (see attached).
So there appears to be some difference between when the app is freshly updated and other times when the battery is charged but the app is not updated.
Obviously I'm not aware of what happens when the app is updated but this does seem to 'fix' the issue for one charge cycle before the high usage returns.
Does this give any clue as to what is going on?

By the way the large figure at the top is the remaining charge so on that day around 56% had been used not 44%
If you look back to Nov 28 2023 in this thread you will see a 'bad' day screenshot of battery drain. On that day the remaining charge was 23% (77% used) but the point is that MD is now top of the usage by some margin.
So for the first charge cycle after an update MD doesn't appear in the list and I get estimated battery life of 60 hours (2 1/2 days) and on others I get half that with MD at the top.
